You cannot add a form.
You can add a variable/column.
Then you click Add New and in the Primary Data Source you select the variable you want to add.

The article I wrote for going from 1.10 to 1.11 (although outdated the procedure does not change). note: if you are on 1.11 and going to 1.12 the upgrader will not run as there is no database structure changes.
