This has been suggested before.It's been recommended that you utilize a third party survey service (like SurveyMonkey) and include a custom url to the site with ticket# name etc in your closing statement. (which you can use the canned responses to do)
Yes, that is the way I do it now, just thought it might be a nice end to a ticket history if you can read the form results right there when looking at the ticket thread. Just a suggestion, this OS Ticket software is amazing!
