1.10.1 is really old, please upgrade to to at least 1.10.6 and see if you still have a problem.

I see 1.12 is out, should I install that one? or go with 1.10.6 first?

If it were me I would upgrade to 1.12 as soon as you can... Once 1.13 or whatever the next major version is numbered comes out it is highly likely that support for 1.10.x will drop off the active list.

I upgraded to 1.12 and I'm still unable to get the emails setting correct. Once I run the diagnostic it come back "error sending email please try again". OSticket is configured and setup on a Synology NAS server, we are using office 365 for our emails.

    dhedwards I'm using osTicket 1.12 with Office 365 too, and the only difference I can see is as @ntozier posted earlier stating that your username and email address are different. If IT@somedomain.ltd and dale.edwards@somedomain.ltd are separate mailboxes, it's going to sulk. Are you able to authenticate using it@somedomain.ltd and an associated password? If it's a shared mailbox, some others have found success with the authentication here: https://forum.osticket.com/d/93787-using-a-shared-mailbox-as-support-mail-office365/14
