Try upgrading your PHP to 5.6There have been a lot of issues reported with versions older than 5.6Also, does not appear to be Feedback or a Suggestion. Moving to Troubleshooting and Problems.

Personally I have ALWAYS has issues with this functionality.  I only click the img icon and drag and drop the image to the ticket that way. (for inline)   or drag and drop it to the attachment bar if I just want to attach it.  I have never had issues adding images this way.
