Hi I have installed a fresh OST v1.10 on Debian Jessie, PHP v7.0.14, Apache v2.4.10 and if I want to create a new ticket, there is no Contact Information forms and if I press on Create Ticket, a new ticket will be created with confirmation "Thank you for contacting us. A support ticket request has been created and a representative will be getting back to you shortly if necessary." but without entering any username, email...How can I debug this error?Thank you

Was a problem on my side, I reinstalled vhost and it's working fine even with php v7.0.14 :)
I have seen a few instances of this and each fix is different it seems. At least this one was different then the last two. For my own knowledge, what is vhost? @[deleted]
I reinstalled system from scratch on virtual host https://en.wikipedia.org/wiki/Virtual_hosting
Ahh, the death of me, I thought vHost was something different.
