In OS ticket  v1.9.12 ,when user creates the ticket there is a option beside subject "  date"  as mention instead of text format so plz suggest me the code or procedure for changing the format    

I haven't the foggiest idea what you are talking about.  Perhaps if you provide a screen capture to high light what you are seeing?

 i had attached the file plz check , i know issue why it is coming . but i don't know how to modify .the script

It appears that you changed your Subject to a time field.Admin panel -> Manage -> Forms -> Ticket Details

yes i done and forget ..now it is resolved oly for ticket got it from mail . what abt  web portal ticket creation

Old tickets are not going to suddenly have a different subject...Unless you go into each ticket and edit the subject.
