I have another doubts,
Created custom form and list for helptopic and working fine,
but custom form field show in below of ticket creation page , how can i show that next to helptopic label in ticket creation page
please suggest me

Move it up on the order in the Form Fields list?
Admin panel -> Manage -> Forms -> Ticket Details
drag the item to where you want it in the order.
