Forms choosing field issue

Dear team

Please help me sort-out this below mentioned issue.

when i try to create Form Fields using Type as choices i cant enter values and when open as user and select that drop down it will showing only one value please help me short-out this issue.

my version - osTicket (v1.11.0-rc1) i am running omn it xammp

I would recommend that you use lists instead of choices fields.

Also your screen shot says: "**Note: if you have more then two choices, use a List instead".

ntozier how to use a list , it is not showing in type coloumn.can you help?
