hi i want to add some new field on os ticket 1.9.1 pls let m know how to do it but that should be also show on account page and update profile page as well as ticket creation page.

There isn't really much more to explain.You go to admin -> manage - formsYou make or add new fields to your Contact Information form.You can also make a list and add it at admin -> manage -> list

ok got it Thanx a ton for this information . some query are below pls help me out on it - 1) i need to add the 2 fields on my contact list and both of them are dropdown but i want that it will dependent on each other one is dependent on other . i mean to say that when i select 1st dropdown according to that 2nd dropdown will populate.2) i want to show these additional fields on tickets.php page of the customer end.

I do not believe that the ui has the capability to have interdependent lists in this manner.You can have two lists, but you cannot have the first list, populate the second one based on how its answered.
