I have osTicket installed and everything works except mail to external clients.

When "users" opens a ticket and writes an external email and submit, they don't get any mail with ticket number,, even if its activated!

osTicket sends alerts and messages to the internal mail accounts on the server notifying about new tickets.

What is wrong?

My website can without problems send out e-mails to external accounts.

Something most likely isn't configured correctly in the admin CP. If you can set me up an account and PM me the details I can take a look.

Have you checked your mail log to see if there is even something attempted to be sent? Usually this can be found in /var/log/mail.log in linux.
