Unable to create ticket

After enter the details when i click create ticket its showing this error(unable to create a ticket . Correct any errors below and try again) unable to create ticket.
But after retype the data when i give click in submit ticket , Then ticket is creating. Advise on this issue.

Please do a recursive search before creating a post. This has been reported and answered many times already.

Thank u so much . I am facing one more issue when my collaborator reply to the ticket its creating as new ticket. There is no change in subject but i am getting duplicate ticket .

Then they are not responding to an alert. They must reply to an existing thread entry in order for it to thread to the Ticket. If it's opening a new ticket then they are sending a new email rather than replying to existing mail. Or you have the Ticket as Closed - Not re-opneable and it's creating a new Ticket.
