That is fetching emails every 19 minutes. So the User replies will only be fetched in batches every 19 minutes. You can up the cron job to 5 minutes and configure your email to fetch more tickets per fetch.

KevinTheJedi I already have the email configured. Here is what is really strange... Yesterday I had the job set to all * * * * * * * basically every minute. Why would it change on its own?
Ask your host that question. This is outside the scope of osTicket. Cron is not controlled by or touched by the software; the cron simply queues a job.
