Change Open.php

How can i change the open.php page and translate the fields label?
Can I also remove the phone number and add other type of fields?

User or Agent open page?
Is the person logged in?
Have they selected their Language?
Have you gone to Admin panel -> Manage -> Forms -> Ticket Details and entered a translation?

Have you gone to Admin panel -> Manage -> Forms -> Ticket Details and entered a translation?

Go to:
Admin panel -> Manage -> Forms -> Contact Information
Click on the Portuguese flag.
Click on the world icon.

Like this (although I am using Spanish)

Click add.
Type in the transalation.
