You cannot create a form that has that exact format in osTicket at this time. You could create a form that was probably relatively close by making a new form, and adding fields for each question.
Got it, I just a mindset thing. Go back and forward found the way to present the information. I wonder if there going to be a type for just HTML that way we can create another type of form. Thanks anyway ntozier
