Ticket Form

Hi. I am back with OS ticket after a while away. I used to have a form (that I am sure was an OS ticket form) The user had to register to send in a ticket.
I can't locate where that form code is, so I can embed it.

If you want to force Users to register for an account you would go to:
Admin panel -> Settings -> Users
See "Registration Required" and "Registration Method"

If you want to add more fields other than the base ones you would go to:
Admin panel -> Manage -> Forms -> Contact Information
