It looks like there is a problem with the phone field. Are you seeing any PHP errors?
I have no PHP error, the phone field is not used.

Please check all available logs (Server logs, Apache error logs, MySQL error logs, PHP error logs, osTicket system logs, Browser Console Logs) for any related errors. Without an error it'd be difficult to track down.
