HTTP 500 error while creating ticket in osTicket V1.14

Hello ... please your help.

I have made the migration of the osticket system V1.9 to V1.14. Up to here everything is fine.

When I create a new ticket I get the HTTP ERROR 500 message.

I have read the previous posts and verified the following:

The LOG file shows me the following message:

[29-Jan-2020 16:19:52 UTC] PHP Fatal error: Uncaught exception 'ObjectNotUnique' with message 'One object was expected; however multiple objects in the database matched the query. In fact, there are 2 matching objects. '

I have checked that there are no duplicate records in the ost_event,ost_file, ost_filter,ost_filter_action, ost_filter_rule,ost_form, ost_help_topic,ost_list, ost_role tables

Everything looks normal but the error persists. I appreciate your collaboration.

Whats the rest of the error message say?

Can you check ost_ticket_status also for dupes?

Hi, thank you very much for responding

I found that the error is in the ost_form table in thetype field since there were two records with the same value there.

This field for what is it used and why the value 'G' in that field if it can appear many times?

Post a screenshot of your ost_form table values and also show what forms are attached to the Help Topic you are creating the Ticket under.

All records appear 1 time except 'G' and 'L51'
