Since we upgrades to 1.12.x, anytime I try to save in the "config" section of a form it stays on the "Loading..." page and does not ever save. I can add items to the forms, but not configure them.

Read the error in the modal...it says: Each choice requires a key and has to be on a new line. (eg. key:value)

All you have to do is make keys for each of your values.

From your screen shot, the information that you input is not in the proper format the error says and @KevinTheJedi told you to use.

Seems like you only have a value.
As a side note, I would recommend using Lists instead.
I did see that error and confirm that each entry is on a separate line. I have this issue even when I have one line listed:

We got it sorted another way.
Created a list - then used the ticket details under machine type with the custom list and integrated it this way.
