My first guess is PHP. Try upgrading to 5.6.x.My second guess would be that there is something prohibiting the .pdf from being created. Such as a security product like SELinux, Mod_security, etc.Consulting your PHP error logs and your Apache error logs and see if anything is being logged when you try to print may point you in the right direction.
