To reproduce, install osTicket 1.10 under PHP 7. After installation, browse to the client's "Open Ticket" form. Contact fields are missing, and the none of the ticket fields show up after selecting the help topic.Full description of the issue is here: https://github.com/osTicket/osTicket/issues/3620

I've seen a few reports of this.1. several of them report that selecting a Help Topic makes the form appear.2. several have reported that their fields were disabled.  Go to Admin panel -> Manage -> Forms.Go though your forms and ensure that the fields are enabled.
