Default Forms - Field Names/API Variables, Custom Form Advanced Filtering

Just have a quick question regarding the default forms. How do I input that data through the API which variable names do i use? Also can you allow searching for tickets using custom fields in custom forms such as is the case using the fields under ticket details.What i am trying to accomplish is essentially create a customer feedback form with a simple question asking the customer to rate the service from 1-5 for ex. This form will go directly to a different department accessible by a select few and bypass the open tickets page. This is all fine and do-able currently however I would like to show only certain fields for that form and take the current fields under ticket details to a new form to do that but still be able to sort or search other tickets not related to customer feedback by those custom fields.

@[deleted], thanks again for your reply. I do have a working version that is successfully posting user information to the custom variables that i have set. I am just a bit confused as to how to access the organizational details through the api. I may need to go back into it and just make sure but from what I know the address field exists several times and still dont know how to store to the phone extension field.Which table contains the emails because i looked at ost_user which contains user info with an email id then looked at ost_ticket_email_info using that id and it has this <cdNwg3BH-g33p0lMZpvfmozp@2c864d1b2c>

Email addresses are stored in ost_user_email in the address column.
