This has happened a few times this past week. One ticket is submitted into the system from a staff user to another. However, multiple new tickets and ticket numbers are created for this one incident. Any ideas what might cause this?
I've seen duplicate tickets created in one of my instances of OSTicket. I always presumed the the employee (client) had pressed submit multiple times. Are you 100% sure that the dupes are a result of a single click of submit?
Unfortunately, I cannot be 100% since I couldn't repeat this issue myself. But I can confirm with 2 specific users (on different levels of literacy) that there was absolute certainty there wasn't a multiple submission.

Yes, the osTicket has a serious BUG regarding this...
But the developers don't want to fix......
