Date Not Showing

Created a custom form for ticket type and the date field does not show once the ticket is created. There there is no data in that field to display.

When the ticket is created the date is in the field but when the ticket is viewed or shown to print with internals checked in the print screen the date field is not showing

If you edit the ticket, is there still data in the date field?

If I go to edit the ticket the date is not there. Shows a blank line
