I'm going to presume that you either fixed the problem, or or it resolved itself some how. I went to the URL provided and opened a ticket just fine. I did not however ever receive an email with my ticket number.
Please check your php error log and let us know what the error is you are receiving.

I have been away and now have some time to resolve the problem. I have version 1.6.
The form seems to work but the reply is in a table to the left, I would like to load it in the center, and use my standard page layout if possible?
An email is now sent to the person completing the form, and a "New ticket Alert" is sent to me. I can see all in the dashboard.
But when I use the system it does not always load the pages I have created, how can I change that?
