I do online photo editing and needed a way for clients to request photo edits and upload pics. osT was the best free option I could find, and with some tweaking it currently looks like this:

I'm desperately waiting for the new username/password account options so I can upgrade my osTicket to that.
I also wish I can figure out how to allow more file uploads and removing the required "subject" field from the form, insteand maybe have the form pull the ticket number and put it in the subject line as "Order #" after its submitted
