Add a field that requires certain value to create new client ticket

I would like to add a field submitonce and require the value to equal 1.

Only when it equals 1 will the ticket be created. I have tried all sorts of things but can't figure out how to make it not create the ticket unless submitonce equals 1.

Any help is much appreciated.
