Mangled php problem with IIS5.1

I had this working perfectly in MAMP and WAMP. Then I moved everything over to test in IIS 5.1 on my work computer

Here's a small cropped jpg of what displays on the page to open a new ticket. I tried modifying the open.inc.php but to avail.

Anyone have any ideas? I've been searching around and playing with the code but haven't found any solutions.

It also happens when loading other pages.

lax minded me forgot to enable short tags in php.ini !@!!!

I remembered that this is also happened with me when the first time I used this osTicket System.
