Since your problem is different then the thread you posted initially it in, I moved your post out and created a new thread.
Regarding your problem:
Table prefix is nothing more then something like 'ost_'. This way if you only have one db, you can easily distinguish the osTicket tables from others.
Hostname is usually just 'localhost'.
Database: if you created a database you had to give it a name. You need to enter that name here.
