http 500

Attempting to run the setup, IIS returns error 500, internal server error. I am unsure where something went wrong..

That's just an access log, doesn't help here at all. 500 error is generic and hints at a deeper fatal error that should be logged in one of the various logs on the server. Like your PHP/PHP-FPM error logs, MySQL/MariaDB error logs, IIS error logs, windows server error logs, etc.

This is the error page I get when I access it from the server itself using localhost.. appears to be a php issue?

Yea I would check php error logs for a related error.
