custom form, using only 2 choices

I'm only using two choices so i don't need to create a custom form but only the first choice is showing and i can't figure out why

my choices are:

key:Caricomm

osTicket Version v1.18.1 (0375576) — Up to date
Web Server Software Apache
MySQL Version 10.7.3
PHP Version 8.2.4

You need to post screenshots or something as I’m not following.

I can reproduce the same if I create a new ticket (running v1.18).

You need to be using v1.18.1 and retest.

LOL I just realized you are both using the same key for both choices. The keys are keys and therefore should be unique.
