Hey, in the email config setting I see the following information about the topic assignment:
Forms associated with the Help Topic will be added to the ticket, but will not have any data.

What does it entail?

You are using a really old version that is no longer supported. I know because the Username and Password fields have been moved to the Basic Authentication Configuration popup in 1.17.x and above.
That part of the help tip should be removed. As you know, we do not add Custom Forms to emailed Tickets (as you cannot fill in fields from emails). Here is an old response about it:
