Check to see if you have any mailer errors in your system logs. It may be falling back to the default MTA or something else to send the mail in which case might have a different name set. You can also see if it’s a saved name in your mail client by viewing the raw mail headers and checking to see if it’s correct there. If it is then you know at some point an email came in with that name and it saved it to your address book or whatever. I know gmail does this sometimes. If that’s the case then you can update the name in whatever mail client you use.
