The code is open source and allows you to modify it to your heart's content. Typically with customizations and mods you’re on your own unless someone has done this before or has an idea and decides to chime in.
You’ll need to modify the client ticket view to filter tickets that do not have private help topics. You’ll also want to modify the code to prevent users from directly accessing a ticket with a private help topic with the ticket id in the URL. And lastly you’ll want to prevent users from searching tickets with private help topics.
