FAQ has no input field

When I try to add an answer to the FAQ their are some input fields missing. Does anyone know how to solve?

Environment details? (see Admin panel -> Dashboard -> Information)

I do not see any missing fields on your screen shot, it looks identical to my production sites.

When clicking on Add FAQ the panel gives a error "Unable to add this FAQ article. Correct any errors below and try again." But there is nothing indicating that their is something wrong. How can I solve this? I can't type anywhere but internal notes. I filled in all the posible inputs but still get this error.
